## **What Is High-Functioning Depression?**

High-functioning depression means you are getting things done on the outside, but feeling low, drained, or disconnected on the inside. You may go to work, take care of kids, keep up with school, and handle daily tasks. From the outside, people may see you as capable and put together. Inside, it can feel like you are walking through mud every day.

Many people think depression always looks like staying in bed, crying a lot, or not being able to function. That does happen for some, but not for everyone. High-functioning depression can sit under a smile, a full calendar, and long to-do lists. Perfectionism, people-pleasing, and high achievement can hide the pain, especially for women who are juggling work, parenting, or caregiving.

Research over the last several years has shown that many people with depressive symptoms keep working and caring for others. Because they seem fine on the outside, their symptoms are often missed or explained away. A [national survey](https://aspe.hhs.gov/reports/work-focused-interventions-depression-final-report-0) found that people with depression report substantially more presenteeism, meaning working while unwell, than people without it, which is part of why the condition so often goes unnoticed. When someone keeps showing up and getting things done, it’s easy for the people around them (and for them) to assume everything is fine. As an anxiety and depression therapist, I look closely at these more subtle signs and help you make sense of what you are feeling, even if no one around you sees anything wrong.

## **Signs You May Be Dealing With High-Functioning Depression**

High-functioning depression can show up in different parts of your life.

### **Emotional Signs**

- A low or flat mood most days
- Feeling numb or checked out, even during “good” moments
- Guilt that you are not enjoying life enough
- Irritability or a short fuse over small things

### **Cognitive Signs**

- Constant self-criticism and harsh inner talk
- Racing thoughts at night when you finally slow down
- Trouble feeling proud of your wins; they never feel “good enough”
- Feeling like an impostor, waiting to be “found out”

### **Physical Signs**

- Changes in sleep, like trouble falling or staying asleep, or needing too much sleep
- Changes in appetite, such as eating more or less than usual
- Frequent headaches or body aches with no clear cause
- Moving through your day on autopilot, doing tasks but not feeling present

- People with high-functioning depression are often seen as the “strong ones.”

- Others lean on you, but you rarely open up.

- You still show up at work, family events, or social activities, but feel more distant or disconnected inside.

High-functioning anxiety often overlaps with depression. You might:

- Worry most of the day
- Feel restless or on edge
- Push yourself to be perfect to avoid criticism or conflict

## **Why High-Functioning Depression Often Goes Unnoticed**

There are many myths that keep high-functioning depression hidden. You might tell yourself, “If I am productive, it can’t be depression,” or “I just need to try harder.” These thoughts make it easy to dismiss your symptoms as personal flaws, not signs of a mood problem.

Outside success can hide a lot of pain. If your work looks good, your kids are fed, and you are keeping up with daily life, friends and even doctors may not ask about your mood. You might also be very skilled at saying “I’m fine” and changing the subject.

Gender roles and cultural expectations can add pressure to keep pushing through. Women and caregivers are often praised for being selfless and strong. Asking for help can feel like failing. Many people under report mood symptoms for this reason. They see feeling low as something they “should” handle alone, instead of a health concern that deserves care.

**Leaving high-functioning depression untreated can have real costs over time, including:**

- Burnout and exhaustion
- Growing distance or conflict in relationships
- Turning to alcohol, food, or scrolling to numb out
- Symptoms worsening into more severe depression

## **Practical Steps to Manage High-Functioning Depression**

If part of you wonders whether something deeper is going on, that matters. You do not have to diagnose yourself, but you can start to pay closer attention.

**Helpful self-check steps include:**

- Tracking your mood and energy each day with a few words or a simple 1-to-10 scale
- Noting changes in sleep, appetite, and motivation
- Watching for patterns, like feeling worse at certain times of day

**Cognitive behavioral therapy (CBT) looks closely at unhelpful thoughts. You can start by noticing thoughts like:**

- “I should be coping better.”
- “Everyone else manages this, so I am just weak or lazy.”
- “If I slow down, everything will fall apart.”

**Once you catch these thoughts, you can ask:**

- What is the evidence for and against this thought?
- Is there a more balanced way to see this?

For example, “I should be coping better” might shift to, “I am carrying a lot right now, and it makes sense that I feel worn down.”

**Behavior-based strategies can also help:**

- Break big tasks into smaller steps and focus on one at a time
- Schedule a few short activities that bring even a small sense of pleasure or calm, like a walk, music, or a hobby
- Set basic limits around work hours, caregiving tasks, and technology use so you have some mental space

   ### Dr. Allen Helped Me to Feel More Empowered

 

Dr. Allen has really helped me find my own voice. When I began therapy I would swing between being passive and doing whatever other people wanted me to do to being angry and frustrated. I have been on antidepressants for quite a few years but it wasn’t really working. Through therapy I have learned to listen to my own needs and to speak up. I used to worry that people wouldn’t like me if I didn’t agree with them but when Dr. Allen gave me the support I needed I challenged my fears. I spend a lot less time feeling angry and depressed now and I have really widened my social network. This is how I have always wanted to be but didn’t know how to get there. Dr. Allen has a very reassuring manner and makes you challenge yourself but by using small steps so you feel ready to do it. I have really come out of my shell and would recommend anyone who is feeling depressed to come and talk with her.
